Medical Social Worker (Government Healthcare/ Community, Casework)

Job purpose:

Reporting to the Chief Operating Officer, the Senior Medical Social Worker is responsible for the overall social services, which seek to promote the psychosocial well-being of the residents and address the concerns of family members during the residents’ stay.

Key Responsibilites

• Psychosocial assessment - assessing strength and resilience of the resident, family, and social support systems to help the resident
• Counselling for residents and family members – for situations in which residents suffer from poor mental states (e.g. depression, anxiety) and coping and adjustment difficulties as well as financial issues
• Financial assessment and fund management - identifying and referring cases for financial assistance
• Referral Services and Discharge Planning – working together with medical, nursing and other allied health staff to: (i) assess new cases referred from Agency for Integrated Care or other sources for suitability, (ii) develop and implement the post discharge plan.
• Supervise team of medical social workers and social work assistants.

• Any ad-hoc duties as assigned.

Requirements:

• Min Degree in Social Work / Post Graduate Diploma in Social Work

• Registered member of Singapore Association of Social Workers (SASW) preferred

• At least 5 years of experience

• Salary will commensurate with level of experience
